Speak with Honcho Oozlgurgle in Plane of Disease. He can be found by porting to POD, then taking the first set of tunnels on the left; pass thru these tunnels, turn left and run high up on the mnt side until you reach a set of tents near the wall map. The Honcho is in one of those tents. He asks you to kill the plagued swine. Kill everything in that valley to spawn the plagued swine, kill and LOOT the swine, with every swine you kill, there is a chance Fleshripper will spawn.

Fleshripper can spawn in 2 vallleys so be sure to try both locations:
1. First valley is the one where you get the quest, e spawns at the opposite end of the valley from the Honcho. It takes a bit longer to spawn but does spawn eventually by killing pigs.
2. Second adjecent valley, in the area where Rotwood the Hollow spawns. Look for pigs there (not as many), spawns quicker per pig kill, but pigs have longer respawn timer.

He can by 1-2 grouped, and he no longer sows and runs. Just pull him to the side so you don't get added while killing.

He has some nice drops; see the PoD Loot Table

Note: If you wish to farm Fleshripper for rares, do not turn in the fleshripper claw to Honcho to complete the quest. If you complete the quest, you can't log it again to spawn him. Once Fleshripper spawns, delete quest from quest long, and delete the slayer claw from inventory after you loot.
